Pregnant mothers face a lot of worries during their pregnancy and sometimes they may worry about the health of the fetus. In general, it is necessary to be familiar with any signs and symptoms in this period, so that as soon as you notice the signs, you can think of an effective solution by maintaining control and coolness. Dr. Zahra Seifi, a gynecologist in West Tehran, can give advice and solutions to her patients about ***signs of fetal viability*** so that the patient can pass this period more easily and adapt to its signs and symptoms more easily. Signs of fetal viability Fetal movements start from the 7th or 8th week of pregnancy, but the mother cannot notice these movements until the 20th week of pregnancy. They usually notice these movements from the 18th to the 20th week of pregnancy. If mothers do not notice movements until 24 weeks, there is no need to worry. After this period, 100% of women feel the movement of the fetus. ### The importance of counting fetal movements Counting the baby's movements is important when something happens to the fetus and the doctor has to remove it, so that the fetus has the ability to survive outside the womb. Therefore, it is better for the mother to count the movements of the fetus from 28 to 30 weeks later. ### How to count fetal movements Since most of the movements of the fetus take place at night, the pregnant mother should consume sweet substances after having dinner so that the fetus wakes up, then sleep on the left side and count the movements of the fetus. Within 1 hour, the fetus must have at least 5 movements. If the fetus does not move during the first hour, this test should be continued for another hour, and if these movements do not occur in the second hour, the pregnant mother should consult a doctor, and from this time on, the mother should take care of her child's health. ### Fetal health assessment When going to the doctor to evaluate the health of the fetus, the best gynecologist in Tehran first performs this evaluation with an ultrasound. This ultrasound, which is called biophysical profile, identifies several issues: Ultrasound biophysical profile - Fetal movement: that is, the sonographer counts the fetal movements. - Breathing of the fetus: because the fetus has breathing and non-breathing cycles during movement. The sonographer waits until the fetus reaches the breathing phase and then starts counting. - Fetal tone: If there is a problem, the fetus may become lethargic, lose weight, and the umbilical cord may be wrapped around its neck. - Measurement of amniotic fluid: the amount of amniotic fluid is measured by ultrasound. Amniotic fluid above 8 cm is usually good. From each of these cases, the fetus receives two marks, and if there is no problem, it receives a full mark, which indicates the good condition of the fetus and its health. ### Fetal ECG This test is performed with a fetal heart rate called NST, that is, the heart rate is measured without stressing the fetus. This biophysical test is completed with the NST test and two points are assigned to this test, that is, if the fetus has at least two movements and 15 heartbeats each time within 20 minutes, then this test is also healthy. If the fetus gets a perfect score from these tests, it means that its health is guaranteed for a week. Color Doppler ultrasound This ultrasound is not routinely done for everyone unless the doctor wants to evaluate the development of the fetus. This ultrasound is used in cases where blood supply is considered by the doctor. Using this method, the doctor detects that the fetus has grown or there is an intrauterine growth disorder, or in cases where the doctor may suspect placental adhesion. For example, a patient who has previously performed a uterine goretage or **Cesarean delivery**, the doctor is worried that the placenta may be stuck in that place and during cesarean section, because of this adhesion, he has to take out the placenta and the uterus at the same time. But this operation is very dangerous, so it is better to detect placental adhesion during pregnancy.
